Service Provider Application Process
Joining our Service Provider Network is easy and straightforward:
- Download and Complete EFT Form: Ensure a hassle-free payment process by filling out the Electronic Funds Transfer (EFT) form, available for download in the resources area below.
- Download and Complete IRS Form W-9: For tax purposes, please submit an IRS Form W-9, which you can conveniently available for download in the resources area below.
- Visit the Born Property Management’s Service Provider Application Page by clicking the “Apply Here” button below and upload the EFT and W-9 Forms mentioned in steps 1 & 2 above and any additional appropriate forms.
